Effective Feb 26, 2024, the OINP requires this form to be accurately filled, dated, and signed by applicants and their dependents. Representatives cannot sign on behalf of applicants. The form is mandatory for new applications, and incorrect submissions will be returned. Ontario Immigration also accepts the Pearson Test of English (PTE) Core as proof of English proficiency.
